This is just one way to get people connected with this blog and with my platform. 

This is easy and so helpful and supportive.

If you absolutely do not know an answer just skip it and fill out the rest of the questions.

 If you know someone in a Care Center:

1. Do you have any relatives in a Care Center? What is the relation? 
2. If you answered no, how is this Care Center Resident connected to you? (Friend, Neighbor, etc.)
3. How long have they been in a Care Center? 
4. Have they remained in one single Care Center?
5. How far away is the Care Center from YOUR home?
6. How far away is the closest (in distance) family member to this resident in the Care Center?
7. Is this resident in the Care Center married?
8. What reason are they in the Care Center for? Disease? Disability? Choice? etc. (Expound and share stories.) 
9. How often do you visit this resident?
10. Do they leave the Care Center? (visits, trips, community events, etc.)
11. Would you think of them as "involved in their community?"
12. Do you think your local Care Center advertised and apart of your community's constant support?
13. Do you help and serve the elderly in your community often? How often?
14. Name five (or as many as you can, if you know more or less) different diseases or disabilities elderly might have:

6. Have you ever visited your local Care Center with your club, group, team, youth group? If so, what was the name of this group?
7. Did you enjoy visiting the Care Center?
8. Do you ever consider what you will be like when you are old?
9. What is the first thing that comes to mind when you hear "Old Folks Home" or "Senior Center"?
10. What do you think the number one reason elderly are in Care Centers for?
